Clearance Sheet
Residents/subspecialty residents who are completing training, obtaining medical staff privileges, terminating or taking a leave of absence are required to complete a Clearance Sheet and have it signed by various departments in the Hospitals and School of Medicine. Please obtain your clearance sheet from your program coordinator and read all instructions on the form before beginning the clearance process.
COBRA
COBRA information will be mailed to you no later than two weeks after your termination date with UNC Hospitals. Please be sure the GME office has your current mailing address. If you wish to enroll in COBRA insurance coverage, you must do so within 60 days of your termination date.
Disability Insurance
The group disability insurance policy with Guardian Life Insurance Company can be converted to an individual policy. If you are interested in conversion, contact Mike Waters at 919-489-1720. Application for conversion should be made prior to your termination to receive full benefit of the conversion clause.
